Records and Confidentiality

On your first visit to Student Health Services, you will be asked to complete a Health History Form, if you have not already submitted one. Your medical and immunization history information is requested to assist the medical staff in making informed decisions regarding your healthcare, thus enhancing the quality of health care provided.

You may pick up a Health History Form at Student Health Services or download the form in PDF format to be printed offline. If you choose to print offline, the completed form may be mailed to Student Health Services, P.O. Box 9038, Grand Forks, ND 58202-9038, or faxed to 777-4835.

A confidential medical record is maintained on every person who receives care at Student Health Services. A record of immunizations is requested of all students to comply with state law requirements. All information in the record is confidential.

Access to information contained in your file is limited to you (the patient) and Student Health Services personnel, unless you give written permission to release it, except by court order. All employees sign a confidentiality pledge.
